Students planning to participate in sports must complete the following procedures before they will be allowed to practice or participate:
1. Must have passed five credit classes the previous semester.
2. Be currently enrolled in at least five credit classes for the current semester.
3. Submit a completed current physical form. (Form available on line)
4. Read, sign, and submit the Co-Curricular Participation Rules form. (Form available on line)
5. Submit a completed Sports Registration Card (Form available on line). Please note: HEALTH INSURANCE is required to participate in sports. Proof of insurance including the policy number will be required. School insurance information can be picked up at registration or in the main office.
Submit the completed forms to Mary in the main office. Once processed and approved, a signed sports registration card will be provided to the coaches. This card is the only admittance to practice.
6. NEW for 2009-10 - payment of District 6 Athletic Transportaton Fee is due at the time of sports registration. The fee is $50 per athlete per sport with a maximum of $150 per household per year. Make checks payable to District 6 - Greeley West High School ( Dist 6 - GWHS)
