Kids Care Clinic

KEEPING KIDS HEATHY IN SCHOOL AND READY T0 LEARN


Our KIDS CARE CLINIC, is housed at Centennial Elementary, and serves Dos

Rios and Maplewood Elementary School, as wells. This year our emphasis will be on

asthma care, obesity, and health education - all of which, affect a student s

ability to learn.


Here are some of the services that we provide at the Kids Care Clinic:

* Immunization * Mental Health & counseling

* Well child exams * Medicaid & CHP+ technician on site

* Injuries * Dental Screening

* Sports physicals * and much more .....


All services are for students and their siblings (up to the age of 19 years

old) of Centennial, Dos Rios, and Maplewood.


The Kids Care Clinic Collaborative Team includes: School Nurse, Health

Clerk, Outreach Coordinator, Behavioral Health Consultant, Family

Nurse Practitioner, Medical Assistant, and a Community Outreach

Technician, which assists with Medicaid and Child Health Plan Plus

(CHP+) for children)


SAME DAY APPOINTMENTS AVAILABLE: FOR SICK OR INIURED CHILDREN.

Our Health Advisory team includes members from our sponsoring agencies: Sunrise Community

Health Center,  North Range Behavioral Hlealth,  District 6, Weld County Department of Public Health
